Adults with a Diabetes Self-Management Plan

This indicator shows the percent of persons ages 18 and older with diabetes or prediabetes who report they have worked with a health care provider to create a self-management to help control their diabetes or prediabetes. Women with gestational diabetes are excluded.

Why is this important?

Diabetes is a serious, lifelong condition that can cause damage to many parts of the body, including eyes, kidney, and nerves.  However, people with diabetes or prediabetes can do many things to protect their health and delay further negative outcomes.  Self-management plans provide important information about living with the disease and the lifestyle choices that can best promote health.  Creating a diabetes self-management plan can greatly increase positive health behaviors, outline necessary screening and medical care, and promote better health and quality of life.
