Description

The goal of Let's Beat Diabetes (LBD) is to prevent or delay the onset of diabetes, slow the progression of the disease, and increase quality of life for those living with diabetes in Counties Manukau, New Zealand. LBD believes that the whole society, whole life course, and whole family must be considered when battling diabetes.

LBD has 10 distinct action areas, one of which is to support primary care based prevention and early intervention. In order to satisfy this action area, facilitators were recruited by primary health organizations in Counties Manukau. These facilitators then delivered a 6-week self-management education program to a group of patients with diabetes.

Goal / Mission

The goal of Let's Beat Diabetes is to prevent new and manage existing cases of Type II Diabetes.

Results / Accomplishments

Patients who participated in the self-management education program were evaluated to assess changes in attitudes, behaviors, and various clinical outcomes. Evaluation data was collected pre- and post-intervention, as well as during a 3-month follow-up. Data concluded that, from pre- to post-intervention, there were significant increases in the number of days that participants reported eating at least 3 meals a day (p<0.001), eating breakfast (p<0.004), eating the recommended servings of fruits (p<0.002) and vegetables (p<0.04), exercising for at least 30 minutes each day (p<0.01), and checking their feet (p<0.006). In addition, the 3-month follow-up showed that there was a significant improvement in levels of hemoglobin A1c, falling from 8.4 to 8.0 on average (p<0.04).

An Evidence-Based Practice meets the following HCI criteria:

  1. The program description includes at a minimum: the sponsoring organization, program goals, program implementation steps, and outcomes that have demonstrated program success in achieving the program goal in one or more localities.
  2. The results from an evaluation of the program include quantitative measures showing improvement in the outcome(s) of interest after the implementation of the program (i.e. increase in smoking cessation, not just the delivery of a smoking cessation program). The outcome measure(s) is/are compared at relevant time periods before and after the intervention or program implementation. Alternatively, the evaluation study compares the outcome(s) between an intervention group and an appropriate control group.
  3. The study is of peer-review quality and presents numbers in a scientific manner; measurements of precision and reliability are included (e.g. confidence intervals, standard errors), results from statistical tests show a significant difference or change in the outcome measure(s), and relevant point estimates and p-values are presented. Note: if the results from an evaluation of a program are presented in a scientific manner and the outcome measure is improved compared to the baseline measurement or the control group but the difference is not statistically significant, the practice is classified as effective and not evidence-based.
