Continuity of Health Care Among Adolescents with Special Health Care Needs

Measurement Period: 2020-2021
This indicator shows the percentage of adolescents with special health care needs aged 12-17 years whose doctors usually/always encourage increasing responsibility for self-care and have discussed transitioning to adult health care, changing health care needs, and how to maintain insurance coverage.

Why is this important?

As children with special health care needs grow into adults, continuity of care is essential.  The transition from pediatrician to a health care provider for adults may involve many issues that are best addressed early.  In addition to changes in insurance coverage, maturing children may need to take a more active role in their own health care (e.g. taking medication, following medical advice).  Having the healthcare provider assist in this transition can relieve the patient and their family of much stress.
